Please follow these instructions completely and accurately in order to qualify for the
rebate(s).

To receive a rebate by mail from Torklift International you MUST complete the
following steps:

1.) Buy a qualifying Torklift product listed on this form between 9/22/15 and
1/4/16.
2.) All documents must be submitted at the same time.
3.) This offer is good only in the United States and Canada.
4.) Rebate(s) are only issued in U.S. funds.
5.) Qualifying storage/security part numbers for $20 rebate offer include:

6.) Register part number purchased above for Legendary Lifetime Warranty.

• Fully completed mail-in rebate form. (see form below)
• A copy of your original, dated sales receipt(s) showing the Torklift International

• Original UPC (see example below) cut off the box(es)* from the original product
package. Do not send the shipping label — this will not be accepted.
• All items above must be included with appropriate postage postmarked by
1/7/16 in order for rebate to be honored.
